We are open on Saturdays from 9 am to 12 pm to make appointments only or to pick up any necessary preparations.
We understand that your schedule may change and that emergencies arise. If you are unable to keep your appointment, we ask that you provide us with 24-hour notice. Please extend us this courtesy so that other patients may be seen.
For those patients who wish to pay their bill online, we accept MasterCard, Visa and Discover cards. Patients may also visit www.epayitonline.com to pay their bill.
Medical insurance coverage has become quite complicated. There are many different insurance carriers, with each company having many different plans. Coverage plans can range from those with total freedom of choice and no pre-authorization requirements to those that limit your choice of physician and require pre-authorization by the insurance carrier and/or a primary care physician.
If you have any questions regarding pre-authorization requirements, please feel free to ask at the time of scheduling. If you have any questions or concerns about your coverage that we cannot provide adequate answers for, please call your carrier for details. If at the time of your examination you have been unable to obtain all the necessary clearances that your insurance carrier requires, we will be unable to perform the exam. These major inconveniences for you are of great concern to us; however, they are not rules that we are responsible for, but rules that we must follow as well.
Indemnity policies and Medicare do not require pre-authorization. Patients may schedule appointments either on their own or through their physician. If you do not see your plan listed, please call our office for more current information. Indemnity policy holders require no pre-authorization or primary care referral form.
The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.
